These services are available for youth who qualify for the Home and Community Based Services (HCBS), Severe Emotional Disturbance (SED), or Medicaid Waiver Program.
Wraparound Facilitation & Planning
A team approach for designing and implementing treatment services for youth on the SED Waiver. The team’s work is based on active leadership and participation of the parents or caregivers. The idea is to wrap services around the child and the family in a coordinated manner based on a Plan of Care, the aim of which is to help keep the family unit intact and to provide the necessary treatments a youth may need.
Independent Living Skill Building
This service is for youth on the HCBS/SED Waiver, and is designed to assist them in transition to adulthood with support for self-help, socialization and adaptive skills necessary for greater independence.
Parent Support and Training and Parent Support Group
These are services for the parents/caregivers of youth who are on the HCBS/SED Waiver, providing them with the training/support necessary to ensure active participation of the family in their child’s treatment planning process, and with ongoing implementation and reinforcement of skills learned throughout the treatment process. Training is provided to family members to increase their ability to provide a safe and supportive environment for the child.
